Overview:
Telmisartan is classified as an angiotensin II receptor blocker (ARB) utilized for the management of hypertension (high blood pressure) and various cardiovascular conditions. By inhibiting the action of angiotensin II—a hormone that causes the constriction of blood vessels—it facilitates vasodilation (the widening of blood vessels), ultimately leading to decreased blood pressure.
This long-lasting ARB is particularly favored for its once-a-day dosing, excellent tissue absorption, and beneficial effects on both cardiovascular and kidney health, making it a recommended option for individuals suffering from hypertension, heart conditions, or kidney-related complications.
Indications:
Primary Indications:
Hypertension (High Blood Pressure) – Reduces blood pressure and lowers the likelihood of heart attacks and strokes.
Cardiovascular Risk Mitigation – Provides protection against heart disease in patients at high risk.
Diabetic Nephropathy (Kidney Protection in Diabetes) – Slows the development of kidney damage in diabetic individuals.
Heart Failure (Off-label Use) – May serve as an alternative treatment to ACE inhibitors for patients experiencing heart failure.
Off-Label Indications:
Chronic Kidney Disease (CKD) – Administered to patients with proteinuria to protect renal function.
Stroke & Heart Attack Prevention – Especially beneficial for patients with additional risk factors like diabetes or high cholesterol.
Dosing Instructions:
For Hypertension (High Blood Pressure): Initial Dose: 40mg daily Maintenance Dose: 40-80mg daily Maximum Dose: 80mg daily
For Cardiovascular Risk Mitigation: 80mg once daily (long-term treatment is advised for optimal results)
For Renal Protection in Diabetes or CKD: 40-80mg daily, depending on renal function and blood pressure objectives.
Can be ingested with or without food. To maintain consistent blood pressure, take at the same time every day. Dose adjustments may be necessary for those with significant kidney or liver disease.
Possible Adverse Reactions:
Common: Dizziness, fatigue, diarrhea, mild headache. Less Frequent: Elevated potassium levels (hyperkalemia), hypotension, muscle cramps. Rare (Seek Medical Attention): Severe allergic reactions, renal impairment, significant blood pressure drops (hypotension).
Precautions:
Not suitable for pregnant individuals – May harm the fetus; discontinue immediately if pregnancy occurs. Use cautiously in patients with renal impairment – Regular monitoring of kidney function and potassium levels may be required. Avoid use with potassium supplements or potassium-sparing diuretics – Can dangerously elevate potassium levels. May interact with NSAIDs (Ibuprofen, Naproxen) – Can diminish effectiveness and adversely affect kidney health.
Storage Instructions:
Store in a dry, cool place at room temperature (20-25°C). Keep out of children's reach and protected from direct sunlight.
